At just 17 years old, Moutinho was called up to the main team by manager José Peseiro during the 2004–05 preseason.
He later went back to the Paulo Bento-coached junior squad, though, and played with players like Miguel Veloso and Nani to help the team win the national championship that year.
Moutinho was called up for a Taça de Portugal game at the start of 2005, and he finally played 20 minutes against F.C. Pampilhosa.
On January 23, he made his Primeira Liga debut, playing the full 3-0 victory against Gil Vicente F.C. while donning the #28 jersey that had formerly belonged to Cristiano Ronaldo.
When veteran Ricardo Sá Pinto left the team in 2006–07, Moutinho—then 19 years old—became vice-captain.
He would be chosen captain the next season, becoming the second-youngest in the club’s professional football history, after first captain and associate founder Francisco Stromp, as Custódio and Ricardo both left.
Moutinho was once again consistently present in the 2008–09 season, missing just three league games as Sporting once again ended in second place, following the collapse of a contract with Premier League club Everton.
Additionally, he had the unfortunate distinction of scoring his team’s lone goal in a 1-12 loss on the aggregate against FC Bayern Munich in the Champions League round of 16.

James Rodriguez and Moutinho were both declared to have joined AS Monaco FC on May 24, 2013, for a reported combined sum of about €70 million.
On September 1, he made his formal debut for his new team, starting and providing the assists for both goals in a 2-1 victory against Olympique de Marseille that sent them to the top of Ligue 1.
On September 16, 2014, Moutinho scored the game-winning goal at home to overcome Bayer 04 Leverkusen in Monaco’s first Champions League encounter since 2005.

For an unknown cost, which was reportedly about £5 million, Moutinho signed a two-year contract with English club Wolverhampton Wanderers on July 24, 2018.

2020-Present
On February 2, 2021, in a 2-1 victory over Arsenal, he scored his first goal at Molineux and third of his Wolves career from 30 yards as his team won both games against the North London club for the first time since 1978–1979 that campaign.
